Applicants will be required to submit the following information/documentation:
· Legal Name and DBA of the business seeking licensure
· Physical address of all locations seeking licensure (mailing address if different)
· Name of any individual with an ownership interest
· Biographic and contact information for any individual with an ownership interest (DOB, SSN#, mailing address, email address, telephone number)
· Contact information for registered agent, if applicable
· A copy of a background check from TBI or the FBI for any individual owner, member-owner, or shareholder
· Verification business is registered with the Tennessee Secretary of State, if operating under a legal entity
· Architectural diagram of the physical space, which includes square footage and dimensions of the remaining space (wholesale warehouses must be 1,000 sq. ft. that is solely used for wholesale storage)
· Documentation reflecting how products will be received, inventoried, stored, and packaged (wholesalers only)
· Documentation reflecting the business records storage process that includes information on how records will be store, secured, and applicants plan for maintaining compliance with applicable legal authority (wholesalers only)
· Deed for the premises of the license sought
· If an applicant does not own the property where its premises is located, lease or related documents necessary to show the applicants property rights in the premises
· Certificate of occupancy
· Detailed business plan including documentation pertaining to the capital required to invest in the business (wholesalers only)
· Proof that applicant has the financial means to engage in distribution (documentation of access to at least $750,000, which may include bonds and lines of credit) (wholesalers only)
· Business hours
· The identity and contact information of any person not previously disclosed who is legally responsible for the management of the applicant’s operations
· 57-7-106(b)(1)(A) and 57-7-106(f)
